Group Treatment Information for Patients, Families & Directors
Patients & Families
Dr. Stach with Bridge City Foot & Ankle will be visiting our facility. He offers routine foot care billed to insurance per allowable Medicare conditions (see below) or for $75 per visit. These visits include complete lower extremity evaluation. Blood flow, skin changes, swelling, and foot or ankle deformities will be checked and appropriate recommendations will be made. Sensation testing will also be performed as appropriate. He will perform nail trimmings, ingrown toenail treatments, callus shaving, wound care, and any other conservative treatment option that will benefit the patient. Recommendations for wounds, fungal toenails, skin infections, and callus care will be made as appropriate. Residents will also have the opportunity to discuss footwear. Over the counter shoe inserts, custom made inserts, and steroid injections to address some foot conditions and pain are potential additional services. See attached flyer linked here.
There are certain conditions that Medicare/Insurance will accept for routine Podiatry care. These conditions include:
-Fungal toenails/thickened toenails
-Ingrown toenails
-Hammertoes
-Bunions
-Spurs/arthritis/foot pain/ankle pain
-Diabetes
-Peripheral vascular disease (poor blood flow)
-Peripheral neuropathies affecting the feet (numbness/tingling/burning)
-Kidney Disease
-Celiac disease
-Malnutrition
-Vitamin deficiencies
-Any wound (no matter how small)-on foot, ankle, or leg
-Any foot or ankle pain
Note: We are accepting Medicare Part B, United Healthcare Advantage Plans/Supplements, Optum, Providence, Care Oregon, AgeRight Advantage, Aetna, Regence BCBS, Trillium Health or $75 Fee for Service visits when diagnosis/insurance coverage does not apply. We are not currently enrolled with other Advantage Plans/Private Insurances at this time; however, we are adding more insurance plans in the next few months. Some other insurance companies will cover out of network providers. We can check eligibility with insurance providers and bill out of network in this case.

Upon receiving new patient information as follows, we will set up their chart, verify insurance coverage and schedule them. For each new patient, we will need the following information via email or fax 971-223-0969 one week prior to our visit:
